In Method B you first show the entry "Noord-Holland" when prompted for State and "Zaandam" when prompted for Locality. Subsequently, in the section for creating the server configuration file you show the entries "ST=Zaandam" and "L=Noord-Holland". They appear to be reversed - shouldn't these be "ST=Noord-Holland" and "L=Zaandam"?
